OF PRIVACY PRACTICES
At
CARE N’ HOME Medical Supply we are committed to providing you
with the best service and using your protected health information
responsibly. This Notice of Health
Information Practices describes the personal information we collect and how and
when we use and disclose it. It also describes
your rights as they relate to your protected health information. Your health information, such as your
diagnosis and prescription, are contained in a medical record or file that is
the physical property of CARE N’ HOME Medical Supply.
Uses and Disclosure of Your Health Information
Treatment. We may use and disclose your health
information to provide you with home medical products and related services. We
may share your health information among other healthcare providers, in an
attempt to provide you with better medical service, include your physicians,
nurses, therapists, and other persons or agencies providing health service to
you.
Payment. We may use and disclose health information
about you to entities such as your insurance companies, in the form of bills,
invoices and letters, to receive payment for our services to you.
Public Health. As required by law, we may disclose your
health information to the appropriate public health or legal authorities.
Law enforcement. We
may disclose your health information to law enforcement officers, when
necessary, as required by law.
Marketing. Occasionally we may use your health information for marketing
purposes.
Reminders. We may use your health information
personally or through a third party to remind you of your appointments or the
need for a refill of supplies.
Business Associates. Occasionally, we will
share your health information with rehab centers and the therapists, who also
work to provide you with better healthcare services.
Your Health Information Rights
Although your health
record is the physical property of CARE N’ HOME Medical Supply,
the information is yours. You have the
right to:
- Inspect and copy your heath record, as
provided for in 45 CFR 164.524,
- Amend your health record as provided in
45 CFR 1G4.528,
- Obtain an account of disclosure of your
health information as provided in 45 CFR 164.528,
- Obtain a paper copy of this Notice of
Privacy Practice upon request,
- Request communications of your health
information by alternative means or at alternative locations,
- Request restriction on certain use and
disclosure of your health information as provided in 45 CFR 164.522, and
- Revoke your authorization to use or
disclose your health information, except to the extent that action has
already been taken.
Our Responsibilities to Your Health Information

Medical Supply is required to:

the privacy of your health information

you with this notice of our legal duties and privacy practices with respect to
your health information.

the right to change the terms of this Notice, including its privacy practices.
